Call for Nominations

Call for Nominations: 2004 Social Worker of the Year Award Submission Deadline, Monday, March 15th, 2004

Applicants: Any active duty USPHS Commissioned Officer Social Worker. One awardee will be chosen for the 2004 award. The Chair, Chair-Elect and Secretary of the Social Worker professional Advisory Group (SWPAG) are not eligible to compete.

Nominations: Nominations may be made by civil servants or commissioned Officers who are familiar with the officer's accomplishments.

Criteria: Candidates must demonstrate:

1. A Positive USPHS professional Image;
2. Outstanding Leadership; and
3. must meet one of the following criteria:

• A specific notable accomplishment that impacts the public or the field of public health and/or social work;
• High quality and initiative in leadership;
• Sustained high quality work performance demonstrated by significant accomplishments throughout one's career;
• The application of unique skill or creative imagination to the approach or solution of a problem(s);
• Technical and/or professional contributions that are significant to an Office, Center, Bureau, Agency or Public Health Service Mission; and/or
• Exhibition of great courage in hazardous work or in an emergency.

Please see the attached Submission Package next page and send by Monday, March 15th, 2004 to me by regular mail or fax.

Submission Package:
A complete submission consists of the following three sections:  the cover page, the nomination page and the C.V.

Cover Page
   • Candidate:
        Name, rank and PHS number of candidate
        Supervisor’s name, address, e-mail and phone number
        Candidate’s position title Work address, telephone, facsimile and e-mail
   • Nominator: (If self nominating, state same as above)
        Nominator’s name (can be self-nominating) and position title
        Work address, telephone, facsimile and e-mail
• Nominator Signature_____________________ and date: _______
Citation: Taken from the last paragraph of the nomination page and should encompass key elements and incorporate the submission.

Nomination Page: On the second page (and third page if needed) describe the following:

    • The first paragraph should state the reason for the nomination and mention the areas of practice.

    • The body of the submission must describe how the candidate demonstrates the three criteria mentioned earlier
       and how the candidate has brought about substantial professional impact/results

    • The last paragraph should encompass key elements and incorporate the submission (also used as the citation).

Attach Curriculum Vitae for the nominated individual that includes a brief description of their current assignment.

Selection Process: the Awards Sub-Committee of the SWPAG may be composed of one or more representatives from the following: Current SWPAG Officers (Chair, Chair-Elect, and Secretary); past SWPAG Chairs; and past recipients of the SWPAG Award.

 Appointments to the Awards Sub-Committee are made based on active participation in SWPAG activities. This group will review all submitted nomination packets that meet the deadline.

The Awards Sub-Committee will rank the most meritorious nominations and select the awardee. The selection of the recipient for the 2004 Social Worker of the Year Award by the Awards Sub-Committee is final. The awardee may accept his or her award at the Commissioned Officers Association (COA) 2004 Public Health Professional Conference or at the 2004 Uniformed Services Social Work Conference. Transportation and accommodations are the responsibility of the awarded Officer.

Remember, the nomination package must be received on or before Monday, March 15th, 2004.
