2005 HS-PAC - CDR Sylvia Tetzlaff
CDR Sylvia Tetzlaff is the 2005 Chair of the Health Services Professional Advisory Committee, and a
Senior Project Management Officer Consultant in the Knowledge Management Staff, Office of
Science and Engineering Laboratories (OSEL), Center for Devices and Radiological
Health (CDRH), Food and Drug Administration (FDA). Some of her responsibilities
include OSEL Webmaster and Configuration Manager for OSEL Software.
CDR Tetzlaff received her Bachelor of Science Degree in Management/Computer Information Systems from Park University in
1986, while serving as an enlisted Data Processing Technician in the United States Navy. She subsequently received her
commission as Ensign, Medical Service Corps, United States Naval Reserve, in 1988 and served the Navy for a total of
seven years.
Since her entry to PHS in June 1995, CDR Tetzlaff has been assigned to IHS OCAO, HRSA’s Division of Immigration Health,
NIH Center for Information Technology, HRSA’s Office of Pharmacy Affairs, and as DCP’s Webmaster. Her PHS activities
include DC-COA Newsletter Chair/Editor (2000-2001), HS-PAC Communications Subcommittee volunteer web developer and
technical consultant (Jan. 2002- Dec. 2002), and Communications Chair and Webmaster (Jan. 2003- Dec. 2004).
Her awards include the DHHS Secretary’s Award for Distinguished Service in 2003, the
2002 HS-PAC Stanley J. Kissel Jr. Award for
Outstanding Senior HSO, two PHS Commendation Medals, Navy Commendation Medal, Navy Achievement Medal, two PHS Unit
Commendations, Regular Corps Ribbon, PHS Bicentennial Ribbon, Navy Good Conduct Medal, and National Defense Service Medal.
2005 HS-PAC Chair Elect - CAPT Jose Belardo
CAPT Jose Belardo is a member of the HS-PAC’s Recruitment and Retention,
Policy, and Mentoring Subcommittees. He entered the Commissioned Corps of the U.S. Public Health Service in June
1995. Since accepting his Commission as an officer, he has been assigned to the U.S. Department of
Health and Human Service’s Health Resources and Services Administration (HRSA). Currently, he is the
Program Director for the Healthy Tomorrows Partnership For Children Program. The Healthy Tomorrows
Program began in 1989 as a collaborative grant program funded and administered by HRSA’s Maternal and
Child Health Bureau (MCHB) in partnership with the American Academy of Pediatrics (AAP).
Its purpose is to stimulate innovative community-based programs that employ
prevention strategies to promote access to health care for mothers and children
nationwide. Before joining Healthy Tomorrows, CAPT Belardo served as a Program Officer in the Division of Perinatal
Systems and Women's Health. He was the Program Officer for 12 urban and rural
Healthy Start (a national initiative created to reduce infant mortality rates in
targeted areas) sites. CAPT Belardo served in that capacity for five years.
CAPT Belardo has a Master's Degree in Social Work (MSW) from Virginia Commonwealth University (1990),
Master's Degree in Administration (Specialization: Health Care Administration) from the University of
Maryland (1999), and a Juris Doctor (JD) (Specialization: Health Care Law) from the University of
Baltimore School of Law.
Upon completing his MSW, he served as a Medical Service Corp Officer in the
U.S. Army. While in the Army, CAPT Belardo served as the Administrator for several family-centered, community-based
programs at different military installations within the United States. He also served as the Chief
Mental Health Officer while deployed to Somalia, Africa during Operation Restore Hope.
CAPT Belardo resides in Frederick, MD with his wife (Elaine) and three children (Lauren, Jared,
and Morgan ages 9, 7, and 2 years respectively).
